Online Application Process

The application process is simple and can be completed from home. Follow these steps to apply for a certificate:

  1. Register on the Portal: Visit the official e-District or Apuni Sarkar portal and create a user account if you don't already have one.
  2. Select the Service: Log in and navigate to the "Birth & Death Registration" section.
  3. Fill the Application Form: Fill out the online application form with all the required details, such as the date and place of birth/death, parent's names, and address.
  4. Upload Documents: Scan and upload the necessary documents. For births, this includes a hospital discharge certificate, and for deaths, it includes a medical certificate of death.
  5. Submit and Pay: Submit the application and pay the required fee online. You will receive an application number to track the status.

Required Documents

The following documents are typically required for the application:

  • For a Birth Certificate: Hospital certificate of birth, proof of parents' identity and address, and marriage certificate of parents.
  • For a Death Certificate: Medical Certificate of Cause of Death (from the hospital/doctor), and proof of identity of the deceased and the applicant.
  • For Late Registration: If the event is not registered within 21 days, an affidavit and an order from a competent authority may be required.
