How to Apply for a Birth Certificate

The application for a birth certificate should be made within 21 days of the event. The process for both online and offline applications is outlined below:

  1. Online Application: Visit the official CRS portal and register as a new user. Fill in the online application form with the child's details, parents' information, and the place of birth. Upload the necessary documents, such as a hospital discharge summary, parent's Aadhaar card, and proof of address.
  2. Offline Application: You can submit a physical application form to the local registrar's office, which is typically located at the Municipal Corporation, Nagar Panchayat, or Block Development Office (BDO).

  • Hospital discharge summary or a certificate from the medical practitioner.
  • Parent's Aadhaar card and identity proof.
  • Proof of address of the parents.

How to Apply for a Death Certificate

A death must also be registered within 21 days of its occurrence. The process is similar to the birth certificate application and is crucial for legal and financial matters.

  1. Online Application: On the CRS portal, select the death certificate application service. Fill in the deceased person's details, date and place of death, and applicant information. Upload the required documents as listed below.
  2. Offline Application: Submit the application form to the local registrar's office with the required documents.

Required Documents for Death Certificate

  • Death report from the hospital or a medical practitioner's certificate (Form 4A for home death).
  • Cremation or burial ground receipt.
  • Identity proof and Aadhaar card of the deceased person and the applicant.
  • Proof of residence of the deceased.

Delayed Registration

If the application is made after the 21-day period, a late fee is applicable. For registration after one year, a magistrate's order is required to certify the event before a certificate can be issued. It is highly recommended to register births and deaths within the specified time to avoid penalties and a lengthy process.
