Eligibility Criteria

• Permanent resident of Uttar Pradesh.
• Must not possess a ration card issued from another state.
• Should meet income norms under AAY, PHH, or NFSA categories.
• Family members must have Aadhaar linked.

Required Documents

• Aadhaar card of all family members.
• Address proof (electricity bill, rent agreement, etc.).
• Income certificate (if applicable).
• Passport-size photographs.
• Active mobile number.

How to Apply

1. Visit official UP Food & Civil Supplies portal: fcs.up.gov.in
2. Download the correct application form (urban or rural).
3. Fill the form, attach documents, and submit at Tehsil/CSC/Block office.
4. Collect acknowledgment number for tracking.

Add a Member to Ration Card

If you want to add a new family member:

1. Fill the member addition form (available at FCS/CSC).
2. Attach Aadhaar, proof of relation, and other required documents.
3. Submit at Tehsil/Block Office or CSC.
