- Submit Documents: Provide your valid identification and address proof, such as an Aadhaar Card, along with a ration card (if applicable) for verification.
- Diagnosis and Referral: After a preliminary diagnosis, the Arogyamitra will assist with generating a health card or a referral for treatment at a network hospital.
A valid ration card (Yellow, Orange, Antyodaya, or Annapurna) is a primary document for verification. In its absence, a Maharashtra domicile certificate can also be used. For road accident victims, the scheme provides coverage up to ₹1 lakh per person per year, irrespective of their origin.
